Bandits Insurance
- this form allows for 20 players to be registered with your team during the AAA season that runs from March 1st to August 31st each year.
- insurance coverage is for Canada and the U.S.

Bandits Liability Questionnaire
- this form is used for teams that will be participating in contact tournaments. Usually at the Major Peewee and above divisions.
- teams participating in contact tournaments are required to complete both the Bandits Insurance form and the Bandits Liability Questionnaire.
- teams participating in contact tournaments in the U.S. will have an additional premium to pay on their insurance.

Bandits Clothing Order Form
- the Cumberland Bandits AAA program requires players to represent the club by wearing club branded apparel at tournaments and exhibition games.
